Our Mission

Strengthen primary care to recognize and manage frailty with clarity and compassion, honoring dignity, supporting informed choice, and improving outcomes for patients and their families.

Our Purpose

Build a clinician-led center that turns the best evidence on frailty into practical action at the point of care, with step-by-step tools, accredited education, practice pilots, and a national learning community.

What is Frailty?

As we age, our bodies can lose the strength and resilience needed to recover from everyday challenges. Frailty is the medical term for this: when a minor illness, a fall, or even a routine surgery can trigger a much bigger setback than expected. The good news is that frailty is not inevitable. When caught early, the right combination of exercise, nutrition, and care can rebuild resilience, improve surgical outcomes, and in many cases, turn things around.

Up to 48%

of frail older adults reversed their frailty through combination interventions including exercise, nutrition, and cognitive support, showing that decline is not inevitable

Ng et al., The American Journal of Medicine, 2015

71%

of primary care interventions combining strength training with protein supplementation showed significant improvement in frailty, demonstrating that physical resilience can be actively rebuilt

Travers et al., British Journal of General Practice, 2019 (46 studies)

2.3x

higher risk of premature death for frail patients, plus 1.8x higher hospitalization risk, but early identification enables targeted interventions that improve outcomes

Vermeiren et al., JAMDA, 2016 (meta-analysis of 31 studies)
